How This Works

You may use the form below to submit a request to return items or to let us know about order shortages. If an item was shorted, please indicate so using the Reason/Status field. Once you've entered in all of the required information and items, please hit "Submit Request." Fields labeled in bold-blue are required.

After submitting the form, a new window will open with a two-page, printable document containing return information that matches the records we will have on file.

If you are located within 50 miles of our Bridgeview warehouse, the first page is for your files, with a space for our driver to sign and date upon pickup. The second page should be affixed to the merchandise to be returned.

If you are out of town, the first page is for your records and the second is to be sent back with the merchandise. We will initiate the pickup via UPS.
